How do you prefer to plan workouts?

I'm curious to hear how all you other trainers plan workouts for yourself and your clients. Do you use pen and paper, a laptop or mobile device, memorize the entire routine, or some other means? Do you present it to your clients before the workout or during? Is there anything unusual or artistic about your program design?
